Apply with ease using your CAQH profile information. 

1. Provide your CAQH information.
2. Attach a completed W-9 and DOO (if applicable).
   Note: The legal entity name on your W-9 must match the name issued on the IRS letters.
3. On CAQH, authorize Envolve Dental and make sure your profile attestation is recent (within 60 days).
